Indications
Black cohosh is a herbal product made from the root of Cimicifuga racemosa (also called Actaea racemosa). It is traditionally used to ease menopausal symptoms such as hot flushes, night sweats, mood changes and sleep problems. Some studies show a modest benefit, while others show none, so evidence is mixed and limited.
How It Works
The exact way it works is not fully known. It does not appear to act like the hormone oestrogen. It may affect brain chemicals such as serotonin that help control body temperature and mood.
Dosage and Administration
Take as directed on the label or by a registered practitioner, usually once or twice daily with water. It is generally not recommended for more than 6 months without medical review. Do not exceed the stated dose.
Interactions
- Use with other medicines that can harm the liver (for example high-dose Paracetamol, some anti-tuberculosis medicines, Methotrexate) may increase the risk of liver injury.
- It may interact with hormone therapy, Tamoxifen and some cancer medicines; ask your doctor first.
- It may add to the effect of blood pressure medicines.
Contraindications
- Allergy to black cohosh or plants of the buttercup family.
- Liver disease, or past liver problems with herbal products.
- Pregnancy and breastfeeding.
- Hormone-sensitive cancers (such as breast cancer) unless approved by your specialist.
Side Effects
Possible side effects include stomach upset, headache, dizziness, rash, weight gain, breast tenderness and spotting. Rare but serious cases of liver damage have been reported.
Stop taking it and get urgent medical help if you develop yellow skin or eyes, dark urine, pale stools, pain in the upper right side of the abdomen, unusual tiredness or loss of appetite (possible liver injury), or signs of a severe allergic reaction.
Pregnancy and Lactation
Do not use in pregnancy, as it may affect the womb and could cause early labour. It is not recommended while breastfeeding because safety is unknown.
Precautions and Warnings
- See a doctor before use if you have unexplained vaginal bleeding, a history of breast or womb cancer, or liver disease.
- Do not use in children.
- If symptoms do not improve within about 3 months, review with your doctor.
