Indications

This Unani preparation is a traditional so-called blood purifier, usually made as an itrifal (a semi-solid herbal paste in a honey or sugar base) whose main herb is shahtara or fumitory (Fumaria parviflora), combined with myrobalan fruits such as halela, balela and amla, and sometimes other herbs. It is traditionally used for skin complaints thought to come from impure blood, such as itching, pimples, boils, rashes and eczema, and to relieve constipation. Scientific evidence for these uses is limited, and it does not replace proper treatment for skin infections, scabies, allergies or other diseases.

How It Works

Fumitory is traditionally believed to cleanse the blood and help the liver, and laboratory studies suggest it may have mild anti-inflammatory and antimicrobial effects. The myrobalan fruits have a mild laxative and astringent effect on the bowel. Together, the formula is meant to clear waste from the body through the bowel; how this affects the skin has not been well studied.

Dosage and Administration

Take as directed on the label or by a registered Unani practitioner, often at bedtime with water or milk. Do not take more than the label amount, and use it only for the period advised. Not for young children unless a practitioner advises it.

Interactions

Because of its laxative effect, it may reduce the absorption of other medicines; take it at least 2 hours apart from your other medicines. Loose stools may increase the risk of low potassium in people taking water tablets or Digoxin. The honey or sugar base can affect blood sugar in people with diabetes. Tell your doctor or pharmacist about all medicines and herbal products you use.

Contraindications

  • Pregnancy.
  • Diarrhoea, dehydration or bowel blockage.
  • Poorly controlled diabetes, unless a doctor advises it.
  • Known allergy to any ingredient.
  • Young children, unless a practitioner advises it.

Side Effects

Possible side effects include loose stools, stomach cramps, gas and nausea. Long-term use may cause weakness from loss of fluid and salts.

Get urgent medical help if you notice swelling of the face, lips or throat, difficulty breathing, severe diarrhoea with signs of dehydration, blood in the stool, or a rash that spreads quickly, blisters or comes with fever.

Pregnancy and Lactation

Do not use during pregnancy, as it contains laxative herbs. There is not enough safety information for breastfeeding, so use it only after advice from a doctor.

Precautions and Warnings

Skin problems such as scabies, fungal infections, acne and eczema usually need specific treatment; see a doctor if they do not improve within two weeks, spread, or come with fever. People with diabetes should count the sugar or honey content. Do not use laxative preparations for long periods without advice.

Overdose

Taking too much may cause diarrhoea, stomach cramps and dehydration. Drink fluids and seek medical help if symptoms are severe, especially in a child or older person.

Therapeutic Class
