About the Medicine

S-Amlodipine

Indications

S-Amlodipine (levamlodipine) is used to treat high blood pressure (hypertension) and to prevent chest pain caused by narrowed heart arteries (stable angina). It can be used alone or with other blood pressure medicines.

How It Works

S-Amlodipine is the active half of Amlodipine. It blocks calcium from entering the muscle cells of blood vessel walls, which relaxes and widens the vessels. This lowers blood pressure and lets more blood and oxygen reach the heart. Because it is the active form, the dose is about half that of ordinary Amlodipine.

Dosage and Administration

Usual adult dose: 2.5 mg once a day by mouth, which the doctor may increase to 5 mg once a day if needed. Elderly people and those with liver problems may start at a lower dose. Take it at the same time each day, with or without food. Keep taking it even if you feel well, unless the doctor tells you to stop.

Interactions

  • Other blood pressure medicines add to its effect.
  • Ketoconazole, Itraconazole, Clarithromycin, Ritonavir and grapefruit juice can raise its levels.
  • Rifampicin and Carbamazepine can lower its effect.
  • It may raise levels of Simvastatin (high doses should be avoided), Tacrolimus and Cyclosporine.

Contraindications

  • Allergy to amlodipine or similar medicines.
  • Very low blood pressure or shock.
  • Severe narrowing of the aortic valve, or unstable heart failure after a heart attack.

Side Effects

Common effects include ankle swelling, headache, flushing, dizziness, tiredness and palpitations. Some people get stomach upset or gum swelling with long use.

Get urgent medical help for chest pain that is new or worse, fainting, a very fast or irregular heartbeat, severe dizziness, or signs of a severe allergic reaction such as swelling of the face or difficulty breathing.

Pregnancy and Lactation

Use in pregnancy only if the doctor decides it is needed. Small amounts pass into breast milk; discuss with the doctor before breastfeeding.

Precautions and Warnings

  • May cause dizziness at the start: be careful when driving.
  • Use with care in liver disease, heart failure and in elderly people.
  • Check your blood pressure regularly.
  • Do not take with another amlodipine product.

Overdose

Overdose can cause a severe drop in blood pressure, fast heartbeat and collapse. Seek emergency medical help.
