About the Medicine

Testosterone Undecanoate

Indications

Testosterone Undecanoate is a form of the male hormone testosterone. It is used as testosterone replacement therapy in men whose bodies do not make enough testosterone (male hypogonadism), confirmed by symptoms and blood tests. Low testosterone can cause low sex drive, erection problems, tiredness, low mood, loss of muscle and body hair, and weak bones. It is available as soft capsules taken by mouth and as a long-acting injection given by a doctor or nurse. It should only be used under the supervision of a doctor.

How It Works

Testosterone Undecanoate is converted in the body into testosterone, which acts on androgen receptors throughout the body. It restores normal male hormone levels, supporting sexual function, muscle and bone strength, red blood cell production, energy and mood. The undecanoate form is absorbed through the lymphatic system when taken with a fatty meal, or released slowly from the muscle when injected, which allows less frequent dosing.

Dosage and Administration

The dose is set and adjusted by the doctor according to symptoms and blood testosterone levels. Capsules are taken by mouth with a meal that contains some fat, usually in divided doses morning and evening, as prescribed; swallow them whole. The long-acting injection is given slowly into a large muscle by a doctor or nurse at intervals set by the doctor, usually every 10 to 14 weeks after the first doses. Do not change the dose yourself.

Interactions

  • Warfarin and other blood thinners: the effect may increase, so blood clotting tests need closer checks.
  • Insulin and other diabetes medicines: blood sugar may fall, so doses may need adjusting.
  • Corticosteroids and ACTH: increased risk of fluid retention and swelling.
  • Ciclosporin and some other medicines may be affected; tell your doctor about all medicines and supplements.

Contraindications

  • Known or suspected prostate cancer or breast cancer in men.
  • Liver tumours, past or present.
  • High blood calcium linked to cancer.
  • Women, particularly during pregnancy and breastfeeding.
  • Allergy to testosterone or any ingredient (some injections contain castor oil or benzyl benzoate; some capsules contain peanut or soya oil).

Side Effects

Possible side effects include acne and oily skin, increased red blood cell count (which can thicken the blood), weight gain, fluid retention, breast tenderness or enlargement, headache, mood changes, changes in sex drive, hair loss, raised blood pressure, worsening of sleep apnoea, and prostate enlargement causing urinary symptoms. Testosterone treatment reduces sperm production and may lower fertility. Injections may cause pain at the injection site and, rarely, an oily coughing reaction during or just after the injection.

Get urgent medical help if you have chest pain, shortness of breath, coughing fits or fainting during or after an injection, pain or swelling in a leg, sudden weakness or numbness on one side of the body, yellowing of the skin or eyes, or a painful erection lasting more than 4 hours.

Pregnancy and Lactation

Testosterone must not be used by women who are pregnant or breastfeeding, as it can cause male features to develop in a female baby. Women should avoid contact with the medicine.

Precautions and Warnings

  • Regular check-ups are needed, including blood testosterone, blood count (haematocrit), PSA and prostate checks, liver tests and cholesterol.
  • Use with caution in heart, kidney or liver disease, high blood pressure, epilepsy, migraine and sleep apnoea.
  • Not for use to build muscle or improve athletic performance in healthy people; misuse can cause serious harm.
  • Use in boys and adolescents only under specialist care, as it can affect bone growth.
  • Prohibited in competitive sport.

Overdose

Overdose of capsules is unlikely to cause immediate serious harm. Contact your doctor; excess hormone effects such as high red cell count or fluid retention may need checking.
