Bexagliflozin
Indications
Bexagliflozin is a tablet used together with diet and exercise to improve blood sugar control in adults with type 2 diabetes. It is not used for type 1 diabetes or diabetic ketoacidosis.
How It Works
Bexagliflozin blocks a transporter in the kidneys called SGLT2 (sodium glucose co-transporter 2), which normally takes sugar back from the urine into the blood. By blocking it, extra sugar leaves the body in the urine, which lowers blood sugar levels.
Dosage and Administration
The usual adult dose is 20 mg once daily in the morning, with or without food. Swallow the tablet whole. Keep taking it every day even if you feel well, and keep following your diet and exercise plan. It is not recommended when kidney function is very low (eGFR below 30), so your doctor will check your kidneys before and during treatment.
Interactions
- Insulin and sulfonylureas such as Gliclazide or Glimepiride: higher risk of low blood sugar; their dose may need lowering.
- Diuretics such as Furosemide: may increase fluid loss and dizziness.
- Rifampicin and other enzyme inducers: may reduce the effect of Bexagliflozin.
- Lithium: blood levels of Lithium may fall.
Contraindications
Do not use if you are allergic to Bexagliflozin or any ingredient, or if you are on dialysis.
Side Effects
Common side effects include genital yeast infections, urinary tract infections, passing more urine, increased thirst, and dizziness from low blood pressure, especially in older people.
Get urgent medical help for nausea, vomiting, stomach pain, unusual tiredness or trouble breathing (possible ketoacidosis, which can happen even with normal blood sugar), fever with severe pain, tenderness or swelling of the genital or anal area, signs of a kidney infection, fainting, or a severe allergic reaction.
Pregnancy and Lactation
Not recommended during the second and third trimesters of pregnancy, as it may affect the baby's kidneys; talk to your doctor about other treatment. Not recommended while breastfeeding.
Precautions and Warnings
Drink enough fluids and take extra care if you are elderly, take diuretics or have low blood pressure. Your doctor may stop it before surgery, during serious illness or when you cannot eat. Keep the genital area clean. Check your feet daily and report any sore or infection. Avoid heavy alcohol use.
Overdose
Overdose may cause passing large amounts of urine, dehydration and low blood pressure. Contact a doctor or go to hospital for supportive care.
