Cenobamate
Indications
Cenobamate is an anti-seizure (antiepileptic) medicine used in adults to treat focal-onset seizures, which are seizures that start in one part of the brain, with or without spreading. It is used when seizures are not fully controlled by other medicines, and is often added to existing treatment.
How It Works
Cenobamate calms overactive nerve signals in the brain in two ways: it reduces a steady flow of sodium into nerve cells that keeps them firing, and it boosts the effect of GABA, the brain's natural calming chemical. Together these make seizures less likely.
Dosage and Administration
Take exactly as prescribed. Treatment starts at a low dose and is increased slowly every two weeks over several weeks to a maintenance dose set by your neurologist; this slow increase is important to lower the risk of a serious allergic reaction. Swallow tablets whole with water, once daily at the same time each day, with or without food. Do not stop suddenly, as seizures can return or worsen.
Interactions
Cenobamate changes the blood levels of several medicines: it can raise Phenytoin and Phenobarbital levels and lower Lamotrigine and Carbamazepine levels, so doses may need adjusting. It can make hormonal contraceptives less effective. Alcohol, sleeping tablets, strong pain killers and other sedating medicines add to drowsiness. Do not combine with medicines that shorten the heart's QT interval in a similar way without medical advice.
Contraindications
Do not take if you are allergic to Cenobamate, or if you have Familial Short QT syndrome (a rare inherited heart rhythm condition).
Side Effects
- Sleepiness, dizziness and tiredness
- Double vision or blurred vision
- Headache
- Problems with balance and coordination
- Nausea, constipation
Get urgent medical help if you develop a rash with fever, swollen glands, yellow skin or eyes, or feel very unwell (signs of a severe allergic reaction called DRESS), if you have fainting or a racing or irregular heartbeat, or if you have thoughts of harming yourself or sudden changes in mood or behaviour.
Pregnancy and Lactation
There is limited information on use in pregnancy. Tell your doctor if you are pregnant or planning a pregnancy; do not stop treatment on your own, as uncontrolled seizures can harm mother and baby. Use non-hormonal or additional contraception. Ask your doctor before breastfeeding.
Precautions and Warnings
Do not drive or operate machines until you know how Cenobamate affects you. Use with care in liver or kidney disease. Like other anti-seizure medicines it may rarely cause depression or suicidal thoughts; family members should watch for mood changes. Avoid alcohol.
Overdose
Overdose may cause severe drowsiness, dizziness, unsteadiness and heart rhythm changes. Seek emergency medical help immediately.
