Tiemonium Methylsulphate
Indications
Tiemonium Methylsulphate is an antispasmodic (a medicine that relaxes cramping muscles in the gut and other organs). It is used for:
- Painful spasms of the stomach and intestines, including cramps linked to irritable bowel syndrome.
- Colicky pain from the gallbladder, bile ducts, kidneys or urinary tract, such as pain from stones.
- Painful periods caused by cramping of the womb.
How It Works
Tiemonium blocks the nerve signals (acetylcholine) that make smooth muscle in the gut, bile ducts, urinary tract and womb contract. It also acts directly on the muscle. This relaxes the spasm and eases cramping pain.
Dosage and Administration
- Tablets, adults: usually 50 mg two or three times a day. Do not take more than 150 mg in a day unless a doctor advises.
- Injection: given by a doctor or nurse into a vein or muscle for severe spasm, at the dose they decide.
- Children: only as prescribed.
Tablets can be taken with or without food. Use for the shortest time needed and see a doctor if pain lasts more than a few days.
Interactions
Other medicines with anticholinergic effects can add to its side effects, such as dry mouth, constipation and difficulty passing urine. These include some antihistamines, Amitriptyline-type antidepressants, some antipsychotics and other antispasmodics such as Hyoscine Butylbromide. It may reduce the effect of Metoclopramide and Domperidone on the stomach.
Contraindications
- Allergy to tiemonium or any ingredient of the product.
- Closed-angle glaucoma (a type of raised eye pressure).
- Difficulty passing urine, for example from an enlarged prostate.
- Blockage of the bowel or a severely slowed, paralysed bowel.
- Myasthenia gravis (a condition causing severe muscle weakness).
Side Effects
Common: dry mouth, blurred vision, constipation, a fast heartbeat and difficulty passing urine. Some people feel drowsy or dizzy.
Get urgent medical help if you cannot pass urine at all, have eye pain with blurred vision or seeing halos around lights, a very fast or irregular heartbeat, confusion, or signs of a serious allergy such as swelling of the face or throat, a widespread rash or trouble breathing. Also see a doctor urgently if belly pain is severe, keeps getting worse, or comes with fever, vomiting or blood in the stool, as these need checking.
Pregnancy and Lactation
There is limited information on use in pregnancy and breastfeeding. Use only if a doctor decides the benefit is greater than the risk.
Precautions and Warnings
- Use with care in heart disease, fast heart rhythms, overactive thyroid, high blood pressure, reflux disease and in older people.
- It may cause blurred vision or drowsiness; do not drive or use machines if affected.
- Take care in hot weather, as it can reduce sweating and raise body temperature.
- It treats spasm only; unexplained or long-lasting belly pain needs a doctor's assessment.
Overdose
Overdose may cause a very dry mouth, flushed hot skin, wide pupils, blurred vision, fast heartbeat, inability to pass urine, restlessness and confusion. Get urgent medical help.
