About the Medicine

Dotinurad

Indications

Dotinurad is used to lower high uric acid levels in the blood (hyperuricaemia), including in people with gout. Lowering uric acid over time helps prevent gout attacks and the build-up of urate crystals in the joints and kidneys. It is not used to treat a gout attack that has already started.

How It Works

Dotinurad is a uricosuric medicine, which means it helps the kidneys remove more uric acid in the urine. It works by blocking URAT1, a transporter in the kidney that normally takes uric acid back into the blood.

Dosage and Administration

  • Adults: take as prescribed. It is usually started at a low dose (0.5 mg once a day) and increased slowly by the doctor over several weeks, guided by blood uric acid tests.
  • Take once a day at about the same time, with or without food.
  • Drink plenty of water during the day unless your doctor has told you to limit fluids.
  • Do not stop taking it if you have a gout attack; continue and contact your doctor.

Interactions

Few interactions are known so far, but experience with this medicine is still limited. Tell the doctor about all medicines you take, especially other medicines for uric acid or gout (such as Allopurinol or Febuxostat), Aspirin, diuretics, and Pyrazinamide, which can affect uric acid levels.

Contraindications

  • Allergy to dotinurad or any ingredient of the product.

It is generally not suitable for people with severe kidney impairment or existing kidney stones, unless the doctor decides otherwise.

Side Effects

Common: gout flare-ups in the first months of treatment (as uric acid levels change), joint pain, and mild changes in liver tests. Kidney stones can occur with this type of medicine.

Get urgent medical help if you have:

  • Severe pain in the side or back, blood in the urine, or great difficulty passing urine.
  • Yellow skin or eyes, dark urine or severe tiredness.
  • Swelling of the face, lips or throat, difficulty breathing, or a severe skin rash.

Pregnancy and Lactation

Safety in pregnancy and breastfeeding is not established. Do not use it during pregnancy unless the doctor decides it is necessary, and avoid breastfeeding while taking it unless the doctor advises otherwise.

Precautions and Warnings

  • Uric acid levels, kidney function and liver tests are checked regularly.
  • Your doctor may give another medicine (such as Colchicine) for the first few months to prevent gout flares.
  • Tell the doctor if you have kidney disease, a history of kidney stones, or liver problems.
  • Limit alcohol and foods high in purines (red meat, organ meat, some seafood) as advised.

Overdose

There is little information on overdose. If too much is taken, contact a doctor or go to the nearest hospital and take the pack with you.
