Sharbat Faulad
Indications
Sharbat Faulad is a traditional Unani iron syrup (faulad means iron or steel). It usually contains an iron compound together with herbs in a sweet syrup base. It is traditionally used as a tonic for weakness, tiredness and paleness caused by iron deficiency anaemia (a low red blood cell count due to lack of iron). It should be used only when iron deficiency is likely or confirmed, as not every anaemia is caused by a lack of iron.
How It Works
Iron is needed to make haemoglobin, the protein in red blood cells that carries oxygen around the body. When the body lacks iron, fewer and smaller red blood cells are made, causing tiredness, breathlessness and pale skin. Iron from the syrup is absorbed in the gut and used to rebuild haemoglobin and iron stores over several weeks.
Dosage and Administration
Take as directed on the label or by a doctor or registered Unani practitioner, usually after meals to reduce stomach upset. Measure each dose with the cup provided. Iron is absorbed better with vitamin C (for example fruit juice) and less well with tea, coffee, milk or antacids, so keep these at least 2 hours apart. Doses for children are as prescribed.
Interactions
Iron can reduce the absorption of Tetracycline, Doxycycline, Ciprofloxacin and other quinolone antibiotics, Levothyroxine, Penicillamine and some medicines for Parkinson disease such as Levodopa; take these at least 2 hours apart. Antacids, calcium supplements and acid-reducing medicines can reduce iron absorption. Tell your doctor or pharmacist about all medicines and supplements you use.
Contraindications
- Iron overload conditions such as haemochromatosis.
- Thalassaemia or other anaemias not caused by iron deficiency, unless a doctor advises iron.
- Repeated blood transfusions, unless a doctor advises it.
- Known allergy to any ingredient.
Side Effects
Possible side effects include nausea, stomach pain, constipation or loose stools, and dark or black stools (which is harmless and expected with iron). Liquid iron can stain the teeth; rinse the mouth or brush after each dose.
Get urgent medical help if you notice black, tarry stools with stomach pain or vomiting of blood, severe stomach pain, or swelling of the face, lips or throat with difficulty breathing.
Pregnancy and Lactation
Iron needs rise during pregnancy and iron is commonly given at this time, but the dose and product should be chosen by your doctor. Iron is safe during breastfeeding at usual doses. Ask your doctor before using a traditional iron syrup during pregnancy or breastfeeding.
Precautions and Warnings
Have a blood test to confirm iron deficiency before long-term use, especially in Bangladesh where thalassaemia is common. Unexplained iron deficiency in men or in women after menopause may be a sign of hidden bleeding and needs a doctor. People with diabetes should count the sugar content. Do not take other iron products at the same time unless advised.
Overdose
Iron overdose is dangerous and can be fatal, especially in young children. Symptoms include vomiting, diarrhoea, stomach pain, drowsiness and collapse. If a child swallows iron syrup or tablets, seek emergency medical help straight away, even if the child seems well.
Therapeutic Class
Unani preparation (iron tonic, haematinic).
Storage
Store below 30°C in a dry place, away from light and out of the reach of children. Keep the bottle tightly closed. Iron is dangerous to children; keep it locked away.
