Butoconazole Nitrate
Indications
Butoconazole Nitrate is an antifungal vaginal cream used to treat vaginal yeast infections (vulvovaginal candidiasis, often called thrush), which cause itching, burning, soreness and a thick white discharge.
How It Works
Butoconazole is an imidazole antifungal. It blocks the fungus from making ergosterol, an essential part of its cell membrane. Without it the membrane leaks and the Candida yeast cells stop growing and die.
Dosage and Administration
The usual adult dose of the 2% cream is one applicator full (about 5 g) inserted high into the vagina once, preferably at bedtime, or as prescribed. Wash your hands before and after use. Lying down after insertion helps keep the cream in place. Symptoms usually improve within a few days; see a doctor if they have not cleared within 7 days. It can be used during a period, but do not use tampons at the same time.
Interactions
Very little is absorbed into the blood, so interactions with oral medicines are unlikely. The cream contains oils that can weaken latex condoms and diaphragms for up to 72 hours after use, so they may not protect against pregnancy or infections. Avoid other vaginal products (douches, spermicides) during treatment unless advised.
Contraindications
Do not use if you are allergic to Butoconazole, other imidazole antifungals, or any ingredient of the cream.
Side Effects
Side effects are uncommon and usually mild: vaginal burning, itching, soreness or swelling, and occasionally abdominal cramps.
Get urgent medical help if you develop a severe rash, swelling of the face or throat or difficulty breathing. See a doctor promptly if you have fever, chills, pelvic or lower abdominal pain, foul smelling discharge, or symptoms that keep coming back, as these may point to another infection.
Pregnancy and Lactation
Use in pregnancy only on a doctor's advice; doctors often prefer a longer course of another vaginal antifungal. Take care with applicators in pregnancy. Little is absorbed, so use during breastfeeding is unlikely to harm the baby, but ask your doctor first.
Precautions and Warnings
Before using for the first time, see a doctor to confirm the diagnosis. Seek advice if you are under 12, have more than 4 infections a year, have diabetes, HIV or a weak immune system, or may have a sexually transmitted infection. The cream is for vaginal use only; do not swallow.
Overdose
Overdose is unlikely with vaginal use. If the cream is swallowed, contact a doctor or poison centre for advice.
