Halobetasol Propionate
Indications
Halobetasol propionate is a very strong corticosteroid (steroid) cream or ointment. It is used for short-term treatment of severe inflammatory skin conditions, such as psoriasis and eczema (dermatitis), that have not improved with weaker steroid creams. Some products combine it with Tazarotene or Salicylic Acid for psoriasis.
How It Works
It calms the immune reaction in the skin, which reduces swelling, redness, itching and scaling.
Dosage and Administration
Adults: apply a thin layer to the affected skin once or twice a day and rub in gently, or as your doctor advises. Wash your hands afterwards. Treatment is usually limited to 2 weeks, using no more than 50 g in a week, unless a doctor says otherwise. Do not cover the area with a bandage or plastic unless told to. Children under 12 years: not recommended unless prescribed by a specialist.
Interactions
Few interactions are expected when used correctly. Do not use another steroid cream on the same area at the same time. If used on large areas, it may add to the effects of steroid tablets such as Prednisolone.
Contraindications
- Allergy to halobetasol or any ingredient
- Untreated skin infections caused by bacteria, fungus or viruses (such as cold sores, chickenpox or shingles), and scabies
- Acne, rosacea and the rash around the mouth called perioral dermatitis
- Use on the face, groin, armpits or open wounds
Side Effects
Common: burning, stinging, itching or dryness where it is applied. Long or heavy use can cause thin skin, stretch marks, easy bruising, visible small blood vessels, changes in skin colour and acne-like spots. If a lot is absorbed (large areas or long use), it can affect the body's own steroid hormones and raise blood sugar.
Get urgent medical help for swelling of the face, lips or throat or difficulty breathing, spreading redness with fever (a possible skin infection), or blurred vision.
Pregnancy and Lactation
Use in pregnancy only if a doctor advises it, on the smallest area for the shortest time. When breastfeeding, do not apply it on or near the breast; use elsewhere only as your doctor advises.
Precautions and Warnings
- Keep away from the eyes; steroid near the eyes can raise eye pressure (glaucoma) or cause cataract.
- Stop when the skin has cleared; your doctor may switch you to a milder steroid.
- After long use, do not stop suddenly without advice, because psoriasis may flare.
- Not for nappy rash or under a nappy.
Overdose
Applying too much once is unlikely to cause harm. Long-term overuse can cause hormone problems such as a round face, weight gain and high blood sugar; see a doctor. If swallowed, seek medical advice.
Therapeutic Class
Topical corticosteroid (very potent).
Storage
Store below 30°C, do not freeze, and keep the cap tightly closed. Keep out of the reach of children.
