About the Medicine

Eltrombopag

Indications

Eltrombopag is a specialist medicine used to raise a low platelet count. It is used for long-term immune thrombocytopenia (ITP) when other treatments, such as steroids, have not worked well enough; for low platelets in people with chronic hepatitis C so that hepatitis treatment can be given; and for severe aplastic anaemia (a condition where the bone marrow does not make enough blood cells).

How It Works

It acts like thrombopoietin, the body's natural hormone that tells the bone marrow to make platelets. This increases platelet production and lowers the risk of bleeding.

Dosage and Administration

The dose is set and adjusted by a specialist (a haematologist or liver specialist) according to regular platelet counts; take it exactly as prescribed, usually once a day. Take it on an empty stomach, at least 2 hours before or 4 hours after dairy products, antacids, and supplements or foods rich in calcium, iron, zinc, magnesium or aluminium, as these stop it being absorbed. Do not stop taking it suddenly unless the doctor tells you, as platelets can drop and cause bleeding.

Interactions

Antacids, calcium, iron, zinc and magnesium supplements, and milk products greatly reduce its absorption. It can raise the levels of Rosuvastatin and some other statins, so lower statin doses may be needed. Cyclosporine and some HIV medicines can change its level. Tell your doctor about all medicines and supplements you take.

Contraindications

Allergy to eltrombopag or any ingredient. It is used with great caution, if at all, in people with significant liver disease.

Side Effects

Common: headache, nausea, diarrhoea, tiredness, muscle or joint pain and changes in liver blood tests. Get urgent medical help for yellow skin or eyes, dark urine or pain in the upper right side of the tummy (liver problems); pain, swelling or warmth in one leg, chest pain, sudden breathlessness or weakness of one side of the body (blood clots); or any serious bleeding, such as black stools or vomiting blood.

Pregnancy and Lactation

Not recommended during pregnancy; women who could become pregnant should use reliable contraception during treatment and for a period afterwards as advised by the doctor. Do not breastfeed while taking it.

Precautions and Warnings

Regular blood tests for platelets and liver function, and eye checks for cataract, are needed. Avoid injury and activities with a high risk of bleeding until your platelet count is safe. People of East or Southeast Asian descent and those with liver disease often need lower doses.

Overdose

An overdose can raise platelets too high and increase the risk of blood clots, and can harm the liver. Contact a doctor or hospital straight away.

Therapeutic Class

Thrombopoietin receptor agonist (medicine that increases platelets).
