Pemafibrate
Indications
Pemafibrate is used, together with diet and exercise, to lower high blood triglycerides (a type of blood fat) and to improve other blood fat levels in adults with dyslipidaemia, especially when triglycerides are high.
How It Works
Pemafibrate is a selective PPAR-alpha modulator. It switches on genes in the liver that help break down triglycerides and fatty acids, reduces the liver's output of triglyceride-rich particles and raises HDL (good) cholesterol. It is more selective than older fibrates, which may mean fewer effects on the liver and kidneys.
Dosage and Administration
The usual adult dose is 0.1 mg twice daily, morning and evening, with or without food. The doctor may increase it to a maximum of 0.2 mg twice daily depending on response. People with kidney problems may need a lower dose, as prescribed. Continue a low-fat diet and regular exercise.
Interactions
Ciclosporin and Rifampicin must not be used with Pemafibrate. Clopidogrel, Clarithromycin, Fluconazole and some other medicines can raise its levels. Taking it with statins may increase the risk of muscle problems, so the doctor will monitor closely. It may increase the effect of Warfarin.
Contraindications
- Allergy to Pemafibrate.
- Severe liver disease, liver cirrhosis or blockage of the bile ducts.
- Severe kidney disease or kidney failure.
- Gallstones.
- Pregnancy or possible pregnancy.
- Use with Ciclosporin or Rifampicin.
Side Effects
Possible side effects include muscle aches, raised liver enzymes, raised creatine kinase (a muscle enzyme), gallstones, stomach discomfort and rash.
Get urgent medical help for unexplained muscle pain, weakness or tenderness, especially with fever or dark urine, which may be signs of serious muscle breakdown (rhabdomyolysis), or for yellow skin or eyes or severe stomach pain.
Pregnancy and Lactation
Do not use in pregnancy or if you may be pregnant. Avoid breastfeeding while taking Pemafibrate unless a doctor advises otherwise.
Precautions and Warnings
Blood tests for liver function, kidney function and muscle enzymes are needed before and during treatment. Use with care in elderly people and those with kidney problems or thyroid underactivity. If triglycerides do not improve after about 3 months, the doctor may review treatment.
Overdose
There is limited experience with overdose. Contact a doctor or hospital if too much has been taken.
