Spectinomycin
Indications
Spectinomycin is an antibiotic injection used to treat gonorrhoea, a sexually transmitted infection, mainly of the urethra, cervix and rectum. It is especially useful for people who are allergic to Penicillin or Cephalosporins, or when the infection is resistant to other antibiotics. It does not work well for gonorrhoea of the throat, and it does not treat syphilis.
How It Works
Spectinomycin attaches to the bacterial ribosome, the part of the cell that makes proteins, and stops it from working properly. Without the proteins they need, the gonorrhoea bacteria stop growing and are cleared by the body.
Dosage and Administration
Spectinomycin is given by a doctor or nurse as a deep injection into a large muscle, usually the buttock. It is commonly given as a single dose; the exact dose, and whether a second injection site is needed for larger doses, is decided by the doctor. Sexual partners should also be tested and treated, and sex should be avoided until treatment is complete and symptoms have gone. The doctor may give another antibiotic as well to cover chlamydia, which often occurs together with gonorrhoea.
Interactions
No important drug interactions are well known. Tell the doctor about all medicines you take, including other antibiotics.
Contraindications
Do not use in people allergic to spectinomycin. The injection may contain benzyl alcohol, so it is avoided in newborn babies.
Side Effects
Pain and soreness at the injection site are common. Other effects may include dizziness, nausea, chills, fever, itching, rash and reduced urine output.
Get urgent medical help for swelling of the face or throat, difficulty breathing, a widespread rash or fainting soon after the injection. Patients are usually watched for a short time after the injection for this reason.
Pregnancy and Lactation
Spectinomycin is considered an option to treat gonorrhoea in pregnancy, especially for women allergic to Penicillin, when the doctor decides it is needed. Untreated gonorrhoea can harm the baby at birth. Little is known about passage into breast milk; the doctor will advise on breastfeeding.
Precautions and Warnings
Testing for syphilis, HIV and other sexually transmitted infections is recommended, as spectinomycin will not treat them. Return to the doctor if symptoms continue after a few days, as resistant infection is possible. Use with care in people with kidney problems.
Overdose
Overdose is unlikely as it is given by health professionals. Too much may increase side effects; treatment is supportive.
