Tiemonium Methylsulfate
Indications
Tiemonium Methylsulfate is used to relieve painful cramps (spasms) of smooth muscle in:
- the stomach and bowel, such as in irritable bowel syndrome, colic and diarrhoea with cramps;
- the bile ducts and gallbladder (biliary colic);
- the urinary tract, such as kidney stone pain (renal colic) and bladder spasm;
- the womb, such as painful periods (dysmenorrhoea).
How It Works
Tiemonium is an antispasmodic of the antimuscarinic type. It blocks the action of acetylcholine, a natural chemical that makes smooth muscle contract, and also relaxes the muscle directly. This eases spasm and the pain it causes.
Dosage and Administration
Adults, tablets: usually 50 mg two or three times a day, or as prescribed. Swallow with water.
Children: syrup by age or body weight, only as prescribed.
Injection: given by a doctor or nurse into a muscle or vein, usually for severe colic, and then switched to tablets when possible.
It treats symptoms only; if pain is severe, keeps coming back or does not settle, see a doctor to find the cause.
Interactions
Other medicines with antimuscarinic effects, such as some antihistamines, tricyclic antidepressants (for example Amitriptyline), some antipsychotics and Disopyramide, can add to side effects like dry mouth, constipation and difficulty passing urine. Tiemonium can reduce the effect of Metoclopramide and Domperidone. Tell your doctor about all medicines, vitamins and herbal products you take.
Contraindications
- Narrow-angle (closed-angle) glaucoma
- Difficulty passing urine due to an enlarged prostate or other blockage
- Blockage or paralysis of the bowel, or severe ulcerative colitis
- Myasthenia gravis
- Fast heart rhythms (tachycardia)
- Allergy to Tiemonium or any ingredient of the product
Side Effects
Common: dry mouth, thirst, blurred vision, constipation, difficulty passing urine, fast heartbeat, flushing and drowsiness.
Less often: dizziness, headache, reduced sweating and skin rash.
Get medical help at once if you cannot pass urine; have severe eye pain with redness, halos around lights or loss of vision (possible acute glaucoma); have a very fast or pounding heartbeat or confusion; develop swelling of the face or difficulty breathing; or if stomach pain becomes severe, constant or comes with fever or vomiting.
Pregnancy and Lactation
Safety in pregnancy is not well established; use only if the doctor advises, especially in the first 3 months. It is best avoided while breastfeeding, as it may reduce milk supply and pass to the baby.
Precautions and Warnings
- Take extra care with heart disease, high blood pressure, an overactive thyroid, reflux (heartburn), liver or kidney disease, and in older people.
- It reduces sweating, so avoid overheating in hot weather or during exercise.
- Blurred vision or drowsiness can affect driving or using machines.
Overdose
Overdose can cause a very dry mouth, hot dry flushed skin, wide pupils, fast heartbeat, inability to pass urine, agitation or confusion. Go to hospital straight away; treatment is supportive.
