Indications

Flumetasone pivalate is a corticosteroid (steroid) applied to the skin or ear. It is used to calm inflammation, redness and itching in skin conditions such as eczema and contact dermatitis. It is often combined with an anti-infective such as Clioquinol, for skin or outer ear (otitis externa) inflammation where a mild bacterial or fungal infection is also present.

How It Works

Flumetasone is a moderately potent steroid. It reduces the release of substances in the skin that cause swelling, redness and itching, and narrows small blood vessels in the inflamed area. In combination products, the added anti-infective helps control germs.

Dosage and Administration

Cream or ointment: apply a thin layer to the affected area once or twice a day and rub in gently. Ear drops: usually 2 to 3 drops into the affected ear twice a day, as directed on the label or by your doctor. Wash your hands after use, unless the hands are being treated. Do not use for longer than one to two weeks, or on large areas, unless your doctor advises it. Do not cover with tight dressings unless told to.

Interactions

  • Very few interactions are expected when used on small areas of skin.
  • Do not use other skin products on the same area at the same time unless your doctor advises.
  • Ritonavir or Itraconazole may increase steroid effects if large amounts are absorbed.

Contraindications

  • Allergy to Flumetasone, Clioquinol or any ingredient.
  • Untreated skin infections caused by viruses (such as chickenpox, cold sores or herpes), tuberculosis of the skin, or untreated fungal or bacterial infections.
  • Acne, rosacea and the rash around the mouth called perioral dermatitis.
  • Ear drops: a damaged (perforated) eardrum.
  • Children under 2 years, unless a doctor advises.

Side Effects

Side effects are uncommon with short use. They may include burning, stinging, itching or dryness where it is applied. Long use can cause thinning of the skin, stretch marks, visible small blood vessels, lightening of the skin and acne-like spots. Combination products with Clioquinol can stain skin, hair and clothes yellow.

Get urgent medical help if you notice swelling of the face, lips or throat, difficulty breathing, a spreading rash, or signs that an infection is getting worse, such as pus, fever or severe pain.

Pregnancy and Lactation

Use in pregnancy only if your doctor advises, for the shortest time and on small areas. During breastfeeding it may be used on small areas for short periods, but do not apply it to the breasts.

Precautions and Warnings

  • Avoid contact with the eyes; steroids near the eyes can raise eye pressure.
  • Use with extra care on the face, skin folds and the nappy area, and in children.
  • Do not use for undiagnosed rashes.
  • Stop and see a doctor if there is no improvement within a week.

Overdose

Overdose from skin use is unlikely, but heavy or prolonged use over large areas can affect hormone balance. If the product is swallowed or used excessively, contact a doctor.

Therapeutic Class

Topical corticosteroid (moderately potent); often combined with an anti-infective.
